Moving to a lesser intensiveness of care. STEPDown: The day will come when your child/adolescent is ready to leave the Center’s Residential Treatment Program, and return to his or her community and family. The San Diego Center for Children wants this return to succeed! We work closely with you to make sure you have all the tools and services necessary to help your child/adolescent move forward. Several months before your child/adolescent leaves the Center, we will have a team and resources in place that will provide you with guidance and preparation so that you, your family and child feel as comfortable as possible when your child returns home. Among the services available to you and your child are individual and family therapy and intensive case management. Wraparound: We recognize how difficult it can be for you to provide the high level of attention and care your child has been accustomed to receiving in residential care. The situation can be especially hard for the child. Once your child is home with you, your entire family will continue to receive services that will facilitate the transition process. Wraparound provides services 24-hour a day, 7-days a week. It helps you get what you and your child needs including intensive case management, individual/family therapy, crisis intervention, rehabilitative services, and medication management. Wraparound will serve any MediCal eligible family who is receiving services through a residential or foster care program with San Diego Center for Children. To assure that your child’s homecoming is as successful as possible, the Wraparound program can stay involved with you for up to one year or more once your child returns home. To make the process as easy as possible you will be assigned an advocate in the form of a Parent Partner. Your Parent Partner has also raised children with system involvement and can help you navigate the sometimes confusing and frustrating network of child welfare, mental health, and special education. A Wrap Facilitator will help you set up meetings and facilitate healthy and clear communication. We are here for you! |
