Mental Health Systems: BPSR Youth Transition Program

MHS BPSR Vista Clinic Transitional Age Youth (TAY) program is a case management wraparound program designed to provide mental health and rehabilitation services specifically for young adults ages 18-24, diagnosed with serious mental illness. Our TAY program helps youth to achieve independence and self-sufficiency, as it builds upon their individual strengths. BPSR (bio-psychosocial rehabilitation) is a model that treats the whole person. Instead of addressing only the symptoms of mental illness, BPSR helps clients recover from the illness and its consequences. With this focus on recovery, BPSR provides rehabilitation and community integration services that meet the individual needs of each client served.

Services:

Case management, individual, group, and family therapy, psychiatric medication management, assistance with educational and vocational goals, healthcare consultation, assistance with socialization, recreational, and independent living skills goals

Payment Options:

Medi-Cal, Medi-Care

Eligibility:

Between the ages of 18-25, living with a serious and persistent psychiatric illness in the cities of Vista or San Marcos, have Medi-Cal and/or Medicare, or have no insurance
