San Diego Center for Children has recognized a long-standing need in our community for increased support for youth as they transition into adulthood. While this life transition is one of excitement and opportunity for many, it also presents a number of new challenges for emerging adults and their families. And when an emerging adult struggles with behavioral health challenges, learning differences, and/or has history of trauma, becoming an adult can feel even more overwhelming. In order to meet the unique needs of these transition age youth, the Center has expanded its services with the Successful Transition Program. The purpose of the Successful Transition Program is to guide transition age youth in creating a personalized, meaningful plan for the future and building the skills and community connections they need to achieve their goals and well-being.

Comprehensive transition needs assessment, transition action planning, transition coaching, case management, referrals and linkage to community resources
