We offer high quality, low-cost primary health care at five state-of-the-art clinic sites. For your convenience we offer extended evening and weekend hours, same day appointments, pharmacy and laboratory services on-site, and free door to door transportation.Our board-certified or board-eligible medical doctors, physician assistants and nurse practitioners serve all members of the community, regardless of insurance status. Uninsured patients can receive help in enrolling in insurance or may be eligible to pay on a sliding fee scale.

Services:

Medical care for adults and children (free transportation to appointments), family planning, mental health services, dental services, HIV testing, immunizations, and a CHOICES Teen Clinic (free and confidential services for youth 12-19 years old)

Payment Options:

We accept Visa, MasterCard, American Express, Discover, cash and checks. We also accept most insurances. If you have insurance, please bring your insurance cards with you. If you don’t have insurance, we can still help you. Depending on your needs and eligibility, we can help you access no cost programs. If you don’t qualify, we also offer a sliding fee scale.
