Mental Health Systems: Strengths-Based Case Management (North)

Strengths-Based Case Management North provides a wide range of services to assist individuals who suffer from a major mental illness or co-occurring disorders. We provide assistance and care coordination from evaluation to discharge planning. Our recovery centered services provide ongoing interventions, which focus on resolving problems through psychosocial rehabilitation and resource management.

Evaluation, placement assistance, psycho-educational counseling, treatment planning, housing assistance, medication monitoring, care coordination, family support, medical linkage, advocacy

Ages 18-59; Must have a chronic major mental illness and /or co-occurring disorders
