Second Chance: Juvenile Options for Lifelong Transition Program

In 2009, Second Chance launched its Juvenile Options for Lifelong Transition Program (JOLT). The JOLT Program is dedicated to providing a comprehensive custody-to-community reentry model based on evidence based practices with the goal to prevent further involvement with the judicial system and support transition aged youth into adulthood. The JOLT Program provides support for 14-24 year old young adults who are often gang involved or affected, and are transitioning from incarceration back into the community. The program serves high risk, transition aged youth, reentering the community and includes gang intervention activities thus increasing public safety and neighborhood wellness.

South Bay Guidance Wellness Recovery Center

The South Bay Guidance Wellness Recovery Center’s services are based on the principles of psychosocial rehabilitation, with an emphasis on attending to the whole person and re-establishing one’s life as a result of the consequences of mental illness, rather than attending to the symptoms of the illness. We focus on recovery, rehabilitation, and community integration, and are individualized to meet the needs of each client. This center also has three specialized programs: Full Service Partnership Case Management, and targeted services for Transition Aged Youth and Older Adults.

The San Diego Metro Region Career Centers provide individuals with a comprehensive approach to career planning and job search services. We offer assistance tailored to the needs of the job seeker such as career counseling, job search assistance, and more. Our commitment is to improve your skills and help you find a job as quickly as possible.

STEPS Program, Salvation Army

Provides a comprehensive employment/transitional living program for homeless men. Clients must be alcohol and drug free and be willing to work full-time. Services include one-on-one case management; job-related preparation with accompanying workshops; employment counseling involving interest and aptitude testing, screening, case planning, budgeting, career development, and weekly follow-up for 9 weeks regarding employment retention; and active job development and job search assistance. The transitional housing supportive services include food and clothing, grooming facilities and supplies, and transportation in the form of bus tokens/passes. Referrals for other support services are provided if necessary.

Toward Maximum Independence

Toward Maximum Independence (TMI) is a nonprofit agency that advocates and assists individuals with developmental challenges and who are deaf and hard of hearing to be included members of the community. Since 1981 TMI has been working with local regional centers and the California State Departments of Developmental Services, Rehabilitation and Habilitation to achieve its’ mission to support people with developmental challenges to live, work and take their rightful place in society.

Urban Beats

Urban Beats is an innovative artistic expression program for Transitional Age Youth (TAY). Through
creating and delivering artistic productions and TAY-focused social media messaging, Urban Beats aims to enhance self-sufficiency and increase TAY engagement in behavioral health treatment. Urban Beats utilizes strengths identification and artistic self-expression to fight the stigma related to mental illness and promote youth self-empowerment and improve wellness.

Urban Corps

Urban Corps is a locally-based nonprofit that provides a high school education and green job training to young adults. The majority of youth employed at Urban Corps did not succeed in a traditional high school setting and we help them get a second chance to go back to school and develop new skills, while contributing to the overall quality of life in the San Diego area. We provide job training and community service in the fields of conservation and recycling, which will assist youth in becoming more employable while protecting San Diego’s natural resources.
