While this life transition is one of excitement and opportunity for many, it also presents a number of new challenges for emerging adults and their families. And when an emerging adult struggles with behavioral health challenges, learning differences, and/or has history of trauma, becoming an adult can feel even more overwhelming. The purpose of the Successful Transition Program is to guide transition age youth in creating a personalized, meaningful plan for the future and building the skills and community connections they need to achieve their goals and well-being.
