Mental Health Systems: Families Forward

This program works to support high-risk youth and their families to decrease re-arrests, re-hospitalizations, truancy, and residential placement. The program’s goal is to empower the client and his or her family and create self-sufficiency. Families Forward can work with child up to 90 days before discharge.

Services:

Intensive case management, crisis intervention, mental health services, treatment groups, medication management, educational support, resource linkage

Payment Options:

Medi-Cal

Eligibility:

6-21 years old, have Medi-Cal or be involved with CWS or the probation system, at risk for losing placement at home or transitioning back home with a return/discharge date, have a mental health diagnosis and meet medical necessity criteria
